UnitedHealthcare, a diversified Fortune 50 provider of health and well-being, and IBM have collaborated with several Arizona-based primary care physician practices to launch the ‘Patient-Centered Medical Home’ (PCMH) model, a pilot program that supports information technology-driven comprehensive primary care for adults, youth and children.
